Who we are

The Homemakers and Nurses Services Program (HMNS) is a community-based service operated by Toronto Long-Term Care Homes and Services. We offer home­making services to eligible individuals who reside within the City of Toronto.

HMNS provides homemaking services through community agency contracts. We provide homemaking services such as light housekeeping, laundry, shopping, and meal preparation to individuals who require assistance with household activi­ties of daily living, and who have limited financial resources. Our customer service commitment is to enrich the lives of the people we serve.

Who is eligible?

You may be eligible for the program if you are ill, convalescing, elderly or disabled, have a limited income, are medically stable, and live in the City of Toronto.

Who pays for the service?

The HMNS program is funded by the City of Toronto and the Province of Ontario. A financial assessment completed by an HMNS program caseworker will deter­mine your eligibility for service. If you are financially eligible for the program, you may need to pay a portion of the cost of homemaking services.

If you wish to proceed, the Intake Caseworker will link you with a Program Caseworker who will visit you in your home to complete a financial and functional assessment, review your rights and responsibilities, develop a plan of service with you and answer any questions you may have. After this visit, the caseworker will then seek approval for your subsidy and service.

If your subsidy and service are approved, you will receive a copy of your rights and responsibilities, a letter that verifies your eligibility and confirms the name of the agency that will be providing service to you, as well as what service will be provided.

The caseworker wil discuss your service needs with the agency providing service to you, and will work together with the agency on an ongoing basis.

The agency will contact you to schedule the homemaker at a mutually convenient time and day.

The caseworker and agency staff will contact and visit with you to reassess your homemaking needs on a regular basis.
